Patient Information

It is our goal to keep you well informed and comfortable with your treatment needs and options.  Please review the following information regarding our office and contact us if we can be of further assistance.

Scheduling your visit

Once your dentist provides us with the information about your particular situation, we can arrange an appointment time for you.  If you should need to adjust your appointment time, we are happy to accommodate your request; we kindly ask for at least 24 hours advance notice if you need to change your appointment.

Our office is centrally located in Sioux Falls at the corner of west 57th Street and Broadband Avenue, about 1 mile south of the Empire Mall.

Before your first visit

Prior to any treatment, please continue to take your medications as directed. When you arrive for your appointment, we will assist you with completing our registration form.  If you prefer, you may print the registration form, complete it at home, and bring it with you to your appointment.

After your visit

Once your treatment is complete, we will review detailed written instructions about how to care for your tooth.  If you should have questions or concerns after your treatment, please contact us promptly.

Financial resources

Prior to any treatment, we will review the cost of your treatment with you.  For your payment we accept cash, check, Visa, MasterCard, Discover, and American Express.  We can also assist you with financing your treatment through CareCredit.  You can use your existing CareCredit account with us, or we can guide you through the process of setting up a new account.


We are happy to process your insurance claim for you.  We will make every effort to estimate your insurance coverage prior to your treatment; however, we cannot provide any guarantee of coverage for your treatment.  We are in-network providers for Delta Dental.
